As an upper cervical chiropractor, I'm particularly concerned with the alignment of the head and neck - so I ask all my patients to bring in their primary sleeping pillow for evaluation. Most of the time it needs to be replaced, especially if it's a foam pillow; the internal structure breaks down over time and becomes "lumpy". Feather pillows also get compacted over time. Generally speaking, it's a good idea to replace your pillow every year or so; you spend a lot of time there!
The goal is to support the head and neck in a neutral position. "Neutral" for each person is highly individual: it depends on the width of your shoulders and hips, the size of your head, whether you're a side or back sleeper (or both), and the material of the pillow. So no one pillow is perfect for everyone! But I do recommend the Peacefull Pillows, especially for my patients who travel or who have chemical sensitivities. I like the flexible support it offers, and the non-toxic nature of it's construction and contents.
Moral of the story? Have someone look at your sleeping posture! and treat yourself to a better night's sleep... Your body needs the rest, and so do you.
